Make An Appointment

To make an appointment, call (505) 559-6100 to speak with a Call Center Coordinator, who will ask you questions about you and your diagnosis. The Call Center Coordinator will be able to answer any questions you may have regarding your visit.

Once we have your information, you will be asked to assist us in obtaining records from your referring or current physician.

The following information will need to be faxed to The Cancer Center at Presbyterian prior to your initial visit:

History and Physical                                                                                  Office notes
Treatment records
Operative reports
Hospital discharge summaries                                                                  Pathology reports
Lab reports
XRAY and scan reports 
Copy of your insurance card, front and back
 

On the DAY of your visit you will need to bring with you:

Copy of your insurance card                                                                          List of current medications and dosages
Pathology slides
Scan and XRAY films
