2011 Requests from Community 501©3s for 2012 Community Benefit Allocations 

To provide financial resources and support for the benefit of community initiatives which directly improve the health of individuals, families and communities.

Deadline date for submitting proposals: Friday, August 19, 2011.

Funding Source

Funding for these allocations comes from unrestricted investment income of the Presbyterian Healthcare Foundation (PHF). Donor contributions to PHF are not used for these allocations unless specifically designated by the donor.

Requirements

  1. Any New Mexico chartered and registered not-for-profit organization may request funds. Approved funding must be utilized within New Mexico. 
  2. Requests must be health related in nature and must demonstrate a goal, which will help to improve a specific aspect of the health of individuals, families and communities.
  3. Funds may be used for a current project or a capital campaign. Funds may not be used to build endowments, endowment campaigns, or to pay for overhead expenses, salaries or other individual compensation. 
  4. Funding for approved requests will be available on Monday, February 20, 2012. These funds are awarded on an annual basis with no implied promise or commitment for future funding. 
  5. If your organization is funded in 2011, please include, with your request, a letter explaining the usage and benefit to your organization. A site visit may be conducted. 
  6. All funding requests must include a cover letter and attachments addressing the below bullets, not to exceed 7 pages. Submit 1 hard copy of the request or a PDF file along with the letter, outlined in #5 above, via email to Phillis Paschall at ppaschall@phs.org PLEASE NOTE videos, binders, tapes or CDs will not be accepted.
    • A brief history and background of requesting organization 
    • The need for the proposed funding request – preferably a one-year commitment 
    • Expected impact and outcomes 
    • A working budget for the funds requested 
    • One copy of the organization's not-for-profit designation 
    • Proof of compliance with the State of New Mexico Charitable Solicitations Act 
    • If applicable, include latest IRS form 990
    • List of Board of Directors and percentage of board giving annually 
    • One copy each of the most recent two years' audited financial statements

All 2012 requests must be received in the Foundation office no later than 5:00 p.m. on Friday, August 19, 2011.
