Registrations on or before October 19th:

Physicians: $225

Residents / Non-physicians: $125 

Registrations After October 19th:

Physicians: $235

Non-physicians: $135

The registration fee includes refreshments, program syllabus, and lunch and is applicable for full or partial attendance.  You may register online or print the registration form and send it in via fax.  In order to fax in your registration, we must have a signed registration form indicating the method of payment and the credit card number.

Early registrations must be received by October 19th.  There is an additional $10 charge for all registration received after that date.
