Manage Your Chronic Disease- MyCD Program
This program is free to NMRHCA members. Join a free 2 1/2 hour MyCD Workshop held each week for six weeks. Learn from trained volunteer leaders with health conditions themselves. Set your own goals and make a step-by-step plan to improve your health and your life.
Click on this MultiPlan link. Click in the upper right-hand corner, "Search for a Doctor or Facility."
Scroll down to the lower box titled "Back of Card."
