Welcome to Your 2013 Presbyterian Health Plan

As an employee of TriCore Reference Laboratories, you have two benefit plans available to you. TriCore has both and HMO and a PPO plan, the HMO plan covers instate contracted  providers only (with the exception of urgent and emergent services), and the PPO plan covers contracted providers nationwide and also includes out of network benefits.

The following Schedules of Benefits are summaries that describe the copayment amounts that apply to specific types of services. Some benefits require pre-authorization by PHP. Benefits may have limits and certain services are excluded altogether.
