Subacute and Rehabilitation (Skilled Nursing)

459_palliative

While most patients are transferred to the skilled nursing unit from acute care hospitals, direct admits may be made from the patient’s home, the hospital emergency room and community clinics. Patients with non-Medicare insurance, such as Presbyterian Senior Care and Presbyterian Health Plan are eligible for direct admit. Patients with Medicare only need three midnights in order to be eligible for the skilled nursing unit. The unit is hospital-based and offers an array of readily available ancillary services, including:

  • physical therapy
  • occupational therapy
  • speech therapy
  • wound care
  • nutritional support
  • nursing
  • radiation

Additionally, the 1:6 nurse to patient ratio of the skilled nursing unit surpasses that of local community-based skilled providers, which average a nurse to patient ratio of 1:12. Patients are transferred to skilled nursing to receive additional therapy in order to get stronger, allow more time for their wounds to heal, and to receive nutritional support and nursing that cannot be provided in the home.
