Medical Staff Appointment Information
Applying for membership in a Presbyterian Healthcare Services (PHS) Medical/Dental Staff and/or the Presbyterian Provider Group.
Formal applications will be considered for those practitioners meeting the following prerequisites:
- A current unrestricted license to practice medicine/dentistry in New Mexico (A pending application is acceptable)
- Completion of postgraduate training as specified in the facility specific Bylaws where you desire membership
- Professional liability insurance coverage in an amount to qualify under the applicable Presbyterian Healthcare Services Bylaws and Rules and Regulations:

- Qualify the practitioner for coverage under the Medical Malpractice Act of the State of New Mexico, or
- Insurance in the amount of $1,000,000 per occurrence/$3,000,000 aggregate for those practitioners in Albuquerque who do not qualify for coverage under the Medical Malpractice Act of New Mexico, or site specific facility amounts or
- Liability coverage provided under the Public Liability funds of the State as administered under the Risk Management Division or a corresponding fund administered under the Federal Tort Claims Act for Federal employees.
To request an application packet, please contact the Office of Medical Staff Affairs at (505) 841-1430. There is an application processing fee of $225, payable at the time the application is submitted.
Following receipt of a completed application, the evaluation and verification of the information contained in the application shall be accomplished. This process takes approximately 90 days and will include the following:
- Primary verification of medical licensure, education, practice experience, past hospital affiliations, professional liability history and personal references.
- Query to the National Practitioner Data Bank as required by law.
- Presbyterian Healthcare Services and its Medical/Dental Staff agree on the importance of having a drug free environment and may direct an applicant for staff membership to undergo screening for controlled substances.
- When applicable, an interview with the Vice-President of Medical Staff Affairs or his designee may be required.
