How to Become an Agent for Presbyterian

Thank you for your interest in becoming an appointed agent for Presbyterian Health Plan, Inc/Presbyterian Insurance Company, Inc.

At Presbyterian, we view our relationship with our agents as a partnership. We believe our responsibility in that partnership is to provide you with exceptional service in areas of communication, materials, timeliness and education. In order to ensure we meet those expectations, we limit the number of agents that we appoint to represent Presbyterian in the New Mexico marketplace. This ensures we meet your expectations while confirming your willingness to also invest in a long-term partnership with Presbyterian.

To apply for review, you will need to provide us with some general information by completing the Prospective Agent Appointment Interview Form (.pdf). Simply download the Form, answer all questions, attach your business plan as indicated and provide a list of references. You may fax this information to (505) 923-8163, Attn.: Anna Weslowski, or send via e-mail to aweslowsk@phs.org.

All agent appointment requests are reviewed quarterly by Presbyterian's Agent Appointment Council. The Council meets at the end of each quarter (see dates below) to review all requests received within that quarter. Determinations are based upon criteria consistently applied across all individuals requesting an appointment. Once your application has been reviewed, you will be notified in writing within 14 working days of the Council's determination.

Presbyterian will not allow any solicitations of business until the Presbyterian Agent Appointment Council has approved the agent and all appointment paperwork has been filed with the New Mexico Insurance Division in Santa Fe.

Please be advised that Presbyterian can not pay any commissions until all licensing and appointment paperwork are complete as stated in the New Mexico Division of Insurance Code Article 12 Chapter 59A. Commissions will be paid the first of the month following the licensing and appointment completion and will not be paid retroactively.
