Make the Connection, Feel the Difference

The Presbyterian Medical Group Cardiac Rehabilitation program provides individualized prescriptive exercise training, education, and counseling for patients with heart and vascular disease. We work with participants to improve cardiovascular health through exercises that increase aerobic capacity, muscular strength, endurance and flexibility. Our staff of seasoned experts includes physicians, nurses, exercise specialists, an exercise physiologist, dietitians, a certified diabetes educator, and respiratory therapist.

We offer two phases of rehabilitation based on your needs. To participate, please request a referral from your physician and contact the Presbyterian Healthplex for a schedule.

Phase II Cardiac Rehabilitation

A physician referred program for individuals with a recent cardiac event or diagnosis such as heart failure, heart attack, coronary artery bypass surgery, or stent placement. Participants exercise in a physician supervised setting and work on risk factor modification through classroom education with support from a multi-disciplinary staff.

Phase IV - Cardiac Maintenance

Phase IV is a maintenance exercise program for Phase II graduates, or for those with a remote cardiac history, and are considered to be at low risk for problems during exercise.
