Skilled Nursing

The inpatient skilled nursing unit at Presbyterian Kaseman Hospital and the swing bed program at Dr. Dan C. Trigg Memorial Hospital are specialized nursing units providing individualized care and treatment when you have complex medical needs.

You may be transferred to skilled nursing to receive additional therapy in order to get stronger, allow more time for your wounds to heal, and to receive nutritional support and nursing that cannot be provided in the home. While most patients are transferred to the skilled nursing unit from acute care hospitals, you may be admitted directly from your home, the hospital emergency room, or a community clinic, if you qualify.

Skilled Nursing and Rehabilitation at Presbyterian Kaseman Hospital

ATTENTION: Effective August 2, we will no longer be accepting new patients at Presbyterian Kaseman Hospital’s Skilled Nursing Facility.

In the skilled nursing unit our team creates an individualized care plan for you, based on your specific needs. The team works together to design a plan for you while you are hospitalized and then, after you are discharged, coordinates safe care, including primary or specialty outpatient care services. The skilled nursing unit specializes in:

  • Care after orthopedic surgery
  • Care for medically complex patients
  • Management of infectious disease
  • Advanced nutritional care
  • Complex wound care
  • Palliative care
  • Hospice care
  • Nursing support for patients undergoing radiation therapy

As needed, you can also benefit from in-room or in-unit therapies to help you increase your physical activities or strength and improve your self-care. Licensed therapists also provide:

  • Evaluation and treatment of speech, swallowing, and cognitive problems
  • Home equipment and safety awareness training
  • Group therapy
  • Therapeutic gym

Swing Bed Program at Dr. Dan C. Trigg Memorial Hospital in Tucumcari

The swing bed program provides structured, multi-disciplinary care to help you heal and strengthen before going home from the hospital. The program emphasizes an environment that promotes independence and self-care. Depending on your doctor's orders, you may receive physical, occupational, speech and/or respiratory therapy.

What is "swing bed" care?

When a hospital is able to use the same bed for either acute hospital care or skilled nursing care, the bed "swings" between the two levels of care. The program helps you recover when you are no longer acutely ill but need extra time to heal or strengthen before returning home. Swing bed care is often covered by Medicare, Medicaid and most private insurance plans.

What is the difference between acute care and swing bed care?

Acute care is medical treatment for patients who have an acute illness or injury or who are recovering from surgery.

Swing bed care is for patients who need additional therapies or nursing care to fully recover before returning to home.

Who is on the swing bed team?

Dr. Dan C. Trigg Memorial Hospital staff members will meet with you and your family to discuss the plan of care and rehabilitation goals. Team members include:

  • You and your family
  • Doctor and/or nurse practitioner
  • Nurses
  • Social Services/discharge planner
  • Dietary Services
  • Physical, occupational, speech and respiratory therapists

Covered Services

  • Room and board
  • Nursing care 24 hours a day
  • Special meals, provided by Dietary Services, as ordered by the physician
  • Physical, occupational, speech and respiratory therapy as ordered by the physician
  • Medications, medical supplies, laboratory services, and the use of rehabilitative equipment furnished by the program during your stay
  • Services of the activity director
  • Social Services/discharge planner
  • Wound care and IV therapy

What are the qualifications for swing bed services?

  • You must have had a three-day acute care hospital stay within the last 30 days.
  • You must need daily skilled nursing or rehabilitative services for a condition treated during the qualifying stay.
  • There must be a reasonable expectation for clinical or functional improvement.
  • You must have a pay source and Skilled Nursing Facility benefit days available.
