Medication Therapy Management is a program offered to Presbyterian Senior Care (HMO) and Presbyterian MediCare PPO members. It is designed to help you get the most out of your medications by identifying and resolving any medication-related issues. This program is provided at no cost to our members.
What are the benefits of participating?
This program offers an over-the-phone or in-person consultation with a Presbyterian clinical pharmacist. Your personal and confidential consultation includes:
- Reviewing your prescription and over-the-counter drugs, including side effects, drug interactions, and more.
- Addressing concerns you have about your drugs, including anything that may prevent you from taking them as prescribed.
- Possibly reducing your drug costs by providing you and your primary care practitioner with cost-effective alternatives.
Following the consultation, you will be provided with a written summary of the consultation, including what you talked about with the pharmacist and a list of all of your current medications.
What is the time commitment?
The length of the consultation depends on several things, such as the number of medications you are taking, the number of conditions you have, and your concerns about your medications. Most appointments take between 30 and 60 minutes. The pharmacist may also contact you after the appointment to make sure all issues have been resolved.
Who is eligible to participate?
You are eligible to participate if you meet all of the following criteria:
- You have at least 2 of the following chronic diseases:
- High Blood Pressure
- High Cholesterol
- Chronic Heart Failure
- Chronic Obstructive Pulmonary Disease (COPD)
- Alzheimer’s Disease
- Cerebrovascular Disorder, such as Stroke or Transient Ischemia Attack (TIA)
- Anticoagulation for conditions such as Atrial Fibrillation, Deep Vein Thrombosis (DVT), or Pulmonary Embolism (PE)
- You are taking at least 6 medications covered by Medicare Part D that are being used to treat these chronic diseases.
- The total cost of your medications is likely to exceed $3,017 during the calendar year. The total cost of your medications is what you pay in copay or coinsurance plus what Medicare pays on your behalf.

How will I be notified if I am eligible?
If you are eligible, you will be mailed a letter inviting you to participate in the program. Presbyterian identifies our members that are eligible at the beginning of every month, so you may receive the invitation letter at any time during the year. You may also receive an email or a telephone call inviting you to participate.
