Centennial Care Information

Presbyterian is pleased to have been selected by the Human Services Department (HSD) to serve as a Centennial Care managed care organization. Centennial Care is the State's new Medicaid program that is replacing Salud, State Coverage Insurance (SCI) and Coordination of Long-Term Services (CoLTS). We look forward to collaborating with New Mexico's practitioners and providers to implement the State's vision of a modernized and integrated Medicaid program that emphasizes care coordination to ensure that members receive the right care, at the right time, in the right setting. Presbyterian is working to expand the Centennial Care program and is continually looking to create new partnerships that will ensure that members have access to all covered services.

As a part of our ongoing implementation efforts, we recently made available the new Centennial Care Practitioner and Provider Manual. This manual provides comprehensive information about Presbyterian's Centennial Care program and is an extension of your contract with Presbyterian. A printed copy of the manual is available at no cost from our Provider Care Unit, which is open 24 hours a day, seven days a week and can be obtained by calling 505-923-5757 or toll-free at 1-888-977-2333. The most current version of the manual is available online and is updated quarterly or as needed.

Centennial Care Training and Orientation

The Centennial Care Orientation and annual Provider Training provided an overview of the new State-funded Medicaid program, Centennial Care and other related program information. Topics included eligibility, care coordination, self-directed community benefit and specialized breakout sessions for Long Term Care and Behavioral Health providers.

There will be additional training and education sessions throughout the year, so please check this webpage frequently for updates and information.

New Provider Directory

Presbyterian has a new and more detailed Provider Directory that now includes office hours for each practice, facility or practitioner and if you have an open or closed panel. If you are a currently contracted provider and need to update your information, please use the Demographic Form.

Currently Contracted Providers

We value our contracted practitioners and providers and we are pleased to continue our partnerships with each of you to care for Centennial Care members. Many of your agreements have been updated to reflect the transition from Salud! to Centennial Care and was outlined in the amendment to you service agreement. As with our standard process, the contract amendments should have required no further action on your part.

Currently contracted providers may also view the sample amendment that transitioned their current agreement from Salud and SCI to Centennial Care.

Behavioral Health Practitioners and Providers

Presbyterian is pleased to partner with new behavioral health practitioners and providers and to continue existing partnerships with those already in our network. If you are not currently contracted with us, you may have received information in the mail from Presbyterian asking you to contract with us to care for Centennial Care members. Long Term Care and Physical Health Providers

Presbyterian is committed to partnering with long term care providers to ensure that members successfully transition to Centennial Care without experiencing a lapse in coverage or services. Long term care services are an essential component of Centennial Care and critical to ensuring that members remain safe and healthy in the least restrictive setting possible. We look forward to developing strong partnerships with providers.

Join Our Long Term Care Network

If you would like to join our network, you can begin the process today. To prepare for a Presbyterian Centennial Care contract, please review the Contracting and Credentialing Checklist for a list of necessary documents and data and then fill out our Provider Profile Form. By completing the Provider Profile Form and submitting your documentation now, we can expedite the contracting and credentialing process. You can also print and fill out the Credentialing Application and mail it to the address provided on the last page of the document.
