Presbyterian’s physical therapy team can help patients regain or improve their physical function and movement after surgery, an injury, or the effects of a chronic illness. Physical therapists use different types of treatments depending on your specific needs and can work closely with your healthcare provider and other therapy team members to create a personalized rehabilitation plan. Some of the types of treatments used may include therapeutic exercises, stretches, manual therapy, return to sport, balance training, training in the use of assistive devices, as well as patient education so you can continue to maintain or improve your physical function even after the formal physical therapy period is completed.
The Healthplex provides the following types of physical therapy treatments:
Amputation therapy, including phantom limb pain and prosthetic training
Balance and gait therapy
Dizziness/vestibular therapy
Dry needling
Hand therapy
Lymphedema therapy
Motor vehicle accident injury therapies
Orthopedic therapy, including pre- and postoperative therapies for surgeries
Joint replacement therapy
Orthotics fitting and training
Pelvic floor therapy
Solid Ground fall prevention classes
Sports-related post-concussion therapy and therapies for muscle weakness or joint injuries
Neurologic therapy for patients with strokes, Parkinson’s disease, Guillain-Barre syndrome, traumatic brain injuries and more
TMJ (temporomandibular joint) therapy
Various therapies for cancer patients
Various types of manual therapies

You can begin the application process by downloading and completing the following two forms. Be sure to print these forms single-sided, NOT double-sided.
After you've completed your forms, bring them to the front desk at the Healthplex to continue the application process.
