Healthy Solutions is a health coaching program available to members at no additional cost. Coaching is available for members with asthma, coronary artery disease, diabetes and hypertension who meet eligibility criteria. Care coordination

Care coordination helps coordinate your healthcare needs, whether you are in the hospital or at home. Care Coordinators work with you to create a plan to help you get the care you need. This is for members with chronic long-term, complex, or behavioral health needs. The level of care coordination you receive depends on your needs
