Your Presbyterian Member ID Card
After enrollment, you are sent a Presbyterian Medicare Advantage member ID card. You must use your Presbyterian member ID card for any services covered by this plan. Here’s why this is so important: If you use your red, white, and blue Medicare card instead of using your Presbyterian member ID card while you are a plan member, you may have to pay the full cost yourself for services you receive (with the exception of routine clinical research studies and hospice services).
If your Presbyterian member ID card is damaged, lost, or stolen, log in to your myPRES account at phs.org/myPRES and select MyIDCard to print, fax, or email your Presbyterian member ID card.
You can also contact the Presbyterian Customer Service Center for a replacement card.

Remember: While you are a member of our plan you must only use your Presbyterian member ID card and not use your red, white, and blue Medicare card to get covered medical services (with the exception of routine clinical research studies and hospice services). Keep your red, white, and blue Medicare card in a safe place in case you need it later.
