Financial Assistance – Help Made Simple
Many Medicare beneficiaries don’t know that they qualify for money saving programs. Your Presbyterian Health Plan membership includes a personal service that helps you find out if you qualify for Low income Subsidy (LIS)/Extra Help, or Medicaid.*

Healthy Solutions
Healthy Solutions is a health coaching program available to members at no additional cost. Coaching is available for members with asthma, coronary artery disease, diabetes and hypertension who meet eligibility criteria. The program is designed to help you create a plan to live a healthy lifestyle.
Care Coordination
Care coordination helps coordinate your healthcare needs, whether you are in the hospital or at home. Care Coordinators work with you to create a plan to help you get the care you need. This is for members with chronic long-term, complex, or behavioral health needs. The level of care coordination you receive depends on your needs.
