Case Management

Coordinated care to ensure careful planning of medications and treatments for patients with multiple or complex conditions, disabilities, or injuries. Phone: 575-257-8200

Dietary

Our team of dietary specialists will help to identify food that meets your specific healthcare needs. Our dietitian offers ongoing education for people who need a refresher on how to take care of their health with diabetes. Phone: 575-257-8266

The Heritage Program for Senior Adults

The Heritage Program is a behavioral health outpatient treatment program that provides individualized intensive treatment to older adults, including conflict resolutions skills, stress management, assertiveness training, and positive living skills. The program operates Monday through Friday with a licensed multidisciplinary treatment team. Lincoln Community Assistance Program

The Lincoln County Community Assistance Program offers professional counseling assistance to Lincoln County residents experiencing any emotional and mental health crisis. The program is free and provides triaging 24 hours a day, seven days a week through a toll-free hotline. Phone: 1-800-888-3689
