Video: Presbyterian partners to reduce disparities

In this video, Presbyterian’s Community Health team explores how we work to improve health equity among the communities we serve in New Mexico through the Healthy Here Coalition.

The Healthy Here Coalition is an Improving Social Determinants of Health Getting Further Faster (GFF) partnership. GFF partnerships are addressing social determinants of health to advance health equity and prevent chronic disease through community-led innovative culturally responsive strategies.

These strategies include the Wellness Referral Center, where providers can refer patients for classes, and Presbyterian’s Food Farmacy, which connects patients to a weekly supply of healthy food.
