Presbyterian Health Plan’s online Provider Directory includes the most current list of in-network providers and facilities. For covered services, refer to your plan documents.

Presbyterian Health Plan and Presbyterian Insurance Company provider networks are the same for both on and off Exchange membership. Presbyterian does not apply any Exchange-specific geographic restrictions, quality measures, member experience measures, or cost-related measures in selecting practitioners or hospitals to participate in our network.

Medicaid - Centennial Care

Centennial Care is a government-funded program that offers health insurance to low-income individuals, including families with children, individuals with disabilities and the elderly.

Medicare Advantage

Presbyterian Medicare Advantage plans are for individuals who are entitled to Medicare Part A, enrolled in Medicare Part B, and live in New Mexico. Some exceptions apply.

Presbyterian MediCare PPO

Presbyterian Dual Plus is a Medicare Advantage special needs plan (HMO SNP) available to Centennial Care health plan members with Medicare who live in Bernalillo, Sandoval, Torrance, or Valencia County.
