Presbyterian Health Plan expands memory care access for members

Presbyterian Health Plan (PHP), New Mexico’s longest-serving managed care organization, is expanding access to dementia diagnosis and care for members statewide through a new partnership.

A collaboration with Synapticure, a virtual neurology provider, addresses a critical need in New Mexico, where access to neurologists is limited—especially in rural and underserved communities. PHP members will now have timely access to subspecialty neurologists, behavioral health clinicians, care coordinators and a 24/7 support team from the comfort of home.

Services include comprehensive cognitive and behavioral assessments, personalized care plans, medication management, caregiver education and ongoing care coordination. Some members may also qualify for the Centers for Medicare & Medicaid Services (CMS) GUIDE model, which provides additional dementia care resources, including up to 80 hours of respite care for caregivers.
