A LIST OF THINGS YOU NEED TO APPLY FOR CENTENNIAL CARE
Proof of your income
Proof that you are a U.S. citizen or a lawful permanent resident (only the person applying needs to show this proof)
Proof that you live in New Mexico

Call the New Mexico Medicaid Call Center for free at 1-888-997-2583. They can also mail you an application.

Visit your local Income Support Division Field Office.
Even if you don’t qualify for Medicaid, you may be able to get financial help to buy your own insurance coverage.

HOW THE APPLICATION PROCESS WORKS
The New Mexico Human Services Department will process your Centennial Care application. You will be asked to choose a health plan when you apply. Presbyterian Health Plan is proud to be part of New Mexico’s Centennial Care program.
You may change the health plan that provides your Centennial Care benefits during your first 90 days of enrollment. For example, if you chose Presbyterian as your health plan but weren’t assigned to us, you have a chance to make this change within 90 days of your enrollment.
For questions about the status of your application, please contact the New Mexico Medicaid Call Center at 1-888-997-2583.
Once your application has been approved, Presbyterian Health Plan will receive your information. You can then expect to receive an ID card and welcome packet in the mail. Then, a representative will contact you to help you fill out your health risk assessment to help us meet your healthcare needs.
