Presbyterian Healthy Activity Rewards
Presbyterian Health Plan (PHP) offers reward points for completing healthy activities and screenings. Points go into your Turquoise Rewards account and can be used to shop for fun, healthy items in the Turquoise Rewards catalog.
Healthy Activities
Breast Cancer Screening – up to $115
Routine breast cancer screenings are the best tool for detecting breast cancer early, even before symptoms appear. Women, and people assigned female at birth, ages 40 to 74 years old and at average risk for breast cancer should get a mammogram every two years.
Members can get rewarded up to $115 (150 points from Turquoise Rewards and 1,000 points from PHP) for completing a mammogram, and reward points can be used to buy items in the Turquoise Rewards online store. Rewards are added once we process the claim.
Well Care Visit (18-21 years old) – Up to $100
Young adults 18-21 years old should visit a primary care provider or OB/GYN at least once a year. When the visit is completed, members can earn up to $100 (200 points from Turquoise Rewards and 800 points from PHP).
Well-Baby Visits – Up to $155
Give your baby a great start by taking them to eight of their well-baby checkups. When you complete all eight visits, you can earn up to $155 (800 points from Turquoise Rewards and $750 from PHP through our Well-Baby Bonuses program).
Prenatal and Postpartum Visits – Up to $300
Expecting and recently delivered parents can join in the Presbyterian Pregnancy Passport, a no-cost program that offers up to $300 (500 points from Turquoise Rewards and 2,500 from PHP) for going to pre- and post-natal appointments.
Diabetes Screenings – Up to $65
Members living with diabetes need annual A1c screenings, retinal eye exams, and kidney health evaluations. When completed, members can earn up to $65 (400 points from Turquoise Rewards and 250 points from PHP). Rewards are added to the Turquoise Rewards account once we process the claim.
Turquoise Rewards
The Turquoise Rewards program is part of New Mexico's Turquoise Care. As a member, you are already enrolled in Turquoise Rewards. Turquoise Rewards also allows you to earn points for doing certain health activities.
For example, you can earn reward points for:
Attending an annual checkup with your primary care provider
Getting a flu shot
Completing health screenings, like a mammogram, cervical cancer screening, and others
Keeping up to date on your children's immunizations
And many more!

Tracking and Spending Points
Most reward points are awarded within three months after you complete a healthy activity and can be used as soon as they are awarded. We will notify you when you earn your first points. All rewards from Presbyterian Health Plan and Turquoise Rewards are combined into one account.
To track or spend points, log in online and visit the Track page or shop online at www.turquoiserewards.com. You can also call toll-free at 1-877-806-8964 (TTY: 1-844-488-9722), 8 a.m. to 6 p.m., Monday through Friday (except holidays), to place an order or request a printed rewards catalog.
Reward points have no cash value and cannot be converted into cash. Reward points can only be used to shop for items in the rewards catalog. Exclusions and restrictions apply. Reward points cannot be used to purchase alcohol or tobacco.
