As a Centennial Care member, you can earn rewards for taking your annual Health Risk Assessment or your Comprehensive Needs Assessment.

You have two ways to earn rewards after taking your Health Risk Assessment.

  • Earn $10 in Centennial Reward points when you complete your Health Risk Assessment
  • You are entered into a monthly drawing for a $25 Walmart gift card from Presbyterian Health Plan

You have one way to earn a reward after taking your Comprehensive Needs Assessment.

  • You earn $10 in Centennial Reward points when you complete your Comprehensive Needs Assessment

What Are Centennial Rewards?

Centennial Care will reward you for doing something good for your health. You can earn points for making certain healthy choices. If you get certain medical tests or fill certain prescriptions (drugs) that help keep you healthy, you will earn reward points. Spend your rewards points on health, wellness and fitness items.

What is a Health Risk Assessment?

A Health Risk Assessment is a short survey about your healthcare needs. You can complete this survey over the phone, on a paper form that you mail to us, or online at your secure member portal, myPRES. We look at your answers and other information about your health to make sure you get the care you need at the right time and in the right place. This also helps us make sure you get all the benefits and services you are entitled to have.

Where Can I Take the Health Risk Assessment?

If you have just enrolled in Presbyterian Centennial Care or are in Level 1 care coordination, you have several different ways to take your Health Risk Assessment. You may call us to take the assessment over the phone or you can ask us to send you an assessment with a prepaid return envelope. You can also take the assessment online in your secure member portal, myPRES.

More About Centennial Rewards and Health Risk Assessments

When Should I Take My Health Risk Assessment?

You need to take a health risk assessment within 30 days after you enroll in Presbyterian Health Plan. You need to take a new assessment once each year around the anniversary date of your last assessment. For example, if you enrolled June 1, you need to take your first assessment during that month. You need to take your next annual assessment the next year in May, June or July.

What If I Take a Comprehensive Needs Assessment Instead of a Health Risk Assessment?

If you are an existing Presbyterian Centennial Care member in Level 2 or Level 3 care coordination, you will complete a Comprehensive Needs Assessment once or twice each year instead of completing a Health Risk Assessment. You will still get Centennial Reward points once each year for completing your assessment.

How Do I Earn Centennial Rewards?

You earn Centennial Rewards points by making certain healthy choices. Centennial Care Rewards may be subject to certain exclusions and restrictions based on the Healthcare Effectiveness Data and Information Set (HEDIS), as modified by your health plan. HEDIS® is a registered trademark of the National Committee for Quality Assurance (NCQA). Call us at 1-877-806-8964 or TTY 800-659-8331 for more information or if you have questions.

How Do I Track and Spend Centennial Rewards?

In order to track your Centennial Rewards, you need to register for a Centennial Rewards account. Go to the Centennial Rewards website and Register Now, or call 1-877-806-8964.

Spend your reward points on a Centennial Rewards card that can be used at certain stores. You can also spend your reward points on fun, healthy items from the Centennial Rewards Catalog or call 1-877-806-8964 (TTY 800-659-8331) to have one mailed to you.
