PRESious Beginnings

PRESious Beginnings is a perinatal nurse case management program committed to healthy babies and birthing outcomes. Premature births (those occurring before 37 weeks) and low birth-weight (less than 5.5 pounds) are a major problem nationwide. PRESious Beginnings works to prevent pre-term labor by identifying and overcoming risks.

What is the risk screening process?

Highly trained registered nurses assess medical, environmental, psychological, socioeconomic, and lifestyle circumstances. From there, an assessment of pregnancy risk factors is made based on the guidelines established by the American College of Obstetricians and Gynecologists.

Who is eligible for PRESious Beginnings services?

Assessment and case management services are available for Presbyterian Centennial Care members and Presbyterian Health Plan members residing in Bernalillo, Sandoval, Torrance, and Valencia counties. Services are conducted via telephone and available in-person at many obstetric clinics.

What do the nurse case managers do?

Our case managers are highly skilled registered nurses, committed to increasing the gestational age and birth weight of newborn infants, and reducing the number of babies admitted into the Neonatal Intensive Care Unit (NICU). By working to overcome risks early in a woman's pregnancy, our team proactively facilitates safe prenatal care and helps prevent premature births.

Our PRESious Beginnings nurse case managers are available to:

  • Identify early signs of complications and educate women about when to report symptoms to their healthcare provider

  • Provide care for pregnant women, starting early in pregnancy and continuing through the post-partum period

  • Provide increased nursing support, answering questions and addressing concerns throughout one's pregnancy

  • Be resource for pregnancy information between provider appointments

  • Conduct universal risk assessment screenings for all PRESious Beginnings clients

  • Meet the educational needs of pregnant women

  • Provide referrals to resources within the community and at the hospital for needed services

Available Services

  • Guidance, education, and support for pregnant mothers

  • Early pregnancy risk screening for pre-term birth risk factors such as pregnancy-induced hypertension, excessive vomiting, history of pre-term birth

  • Nurse availability in many Presbyterian OB/GYN clinics

  • Referrals to prenatal education classes, doula services, and community services for identified client needs

  • Presumptive Eligibility (PE) and Medicaid Application Assistance (MOSAA)

  • Educational materials for women and their families including:

    • Books, handouts, and DVDs

    • General pregnancy information

    • Risk-factor specific pregnancy information

    • Post-partum materials

    • Infant development information

    • A nurse case manager to answer the many questions that expectant moms have
