Pregnancy and Well-Baby Reward Programs

Pregnancy Passport

Presbyterian Pregnancy Passport is a no-cost program that offers rewards for attending prenatal and post-natal appointments. We want to help you enjoy and understand your pregnancy. We offer:

Gift card incentives for going to your prenatal doctor visits
Care coordination services if you have a high-risk pregnancy
A 24/7 PresRN phone line for help any time of the day or night
Transportation services to and from your doctor visits
Help with making your doctor's appointments

When you complete steps in the program, you can earn gift card incentives. Presbyterian Pregnancy Passport is only available to Presbyterian Turquoise Care members.

Such services are funded in part with the State of New Mexico.

STEP ONE

Complete a prenatal visit with your primary care (PCP)/family doctor, nurse midwife or OB/GYN within the first trimester (Week 1-14) of pregnancy, or within 42 days of joining Presbyterian Health Plan.

Step One Reward: Earn a $150 gift card to buy healthy items for yourself during your pregnancy. Stay well!

STEP TWO

Complete at least 10 prenatal care visits. (Or 80% of the number of visits determined by your provider.) Prenatal providers can be a primary care (PCP)/family doctor, nurse midwife and OB/GYN.

Step Two Reward: Earn a $75 gift card for supplies for yourself and your baby. Prepare for your baby's arrival!

STEP THREE

Complete your postpartum visit within 7-84 days after your baby is born. This visit is for your care with your PCP or OB/GYN.

Step Three Reward: Earn a $100 gift card for newborn items. Congratulations!

Baby Bonuses

Baby Bonuses is a no-cost program that offers rewards for taking your baby to their well-baby checkups. We want to help you enjoy and understand your baby’s growth. We offer:

Gift card incentives for taking your baby to their well-baby checkups
A 24/7 PresRN phone line for help any time of the day or night
Transportation services to and from provider visits
Help making appointments with providers

When you complete the steps in the program, you can earn gift card incentives. Presbyterian Baby Bonuses is only available to Presbyterian Turquoise Care members.

FIRST SIX VISITS

Complete six well-baby visits with your baby’s primary care provider (PCP) before your baby turns 15 months of age.

First Five Visits Rewards: Earn a $10 gift card for each visit to buy healthy items for your baby as they grow. ($50 total)
Sixth Visit Rewards: Earn a $15 gift card for the sixth visit your baby has before the age of 15 months. ($15 total)

SEVENTH AND EIGHTH VISITS

Complete two additional visits with your baby’s primary care provider (PCP) when your baby is between the ages of 16-30 months.

Seventh and Eighth Visit Rewards: Earn a $10 gift card for each visit to buy supplies for your baby. ($20 total)

Once enrolled, please fill out this form after each well-child visit and send it to the above address.

Well-Child Visit Form

We will accept submissions for up to one year after your baby’s birth. Please allow 4-8 weeks to receive your gift card(s).

Turquoise Rewards

Turquoise Care will reward you for doing something good for your health. You can earn points for your good habits. If you get certain medical tests or fill certain prescriptions (drugs) that help keep you healthy you will earn reward points. Participating in our Baby Bonuses can help you earn rewards. Spend your rewards points on health, wellness and fitness items.
