Baby Bonuses is a no-cost program that offers rewards for taking your baby to their well-baby checkups. We want to help you enjoy and understand your baby’s growth. We offer:

  • Gift card incentives for taking your baby to their well-baby checkups

  • A 24/7 PresRN phone line for help any time of the day or night

  • Transportation services to and from provider visits

  • Help making appointments with providers

When you complete the steps in the program, you can earn gift card incentives. Presbyterian Baby Bonuses is only available to Presbyterian Centennial Care members.

FIRST SIX VISITS

Complete six well-baby visits with your baby’s primary care provider (PCP) before your baby turns 15 months of age.

  • First Five Visits Rewards: Earn a $10 gift card for each visit to buy healthy items for your baby as they grow. ($50 total)

  • Sixth Visit Rewards: Earn a $15 gift card for the sixth visit your baby has before the age of 15 months. ($15 total)

SEVENTH AND EIGHTH VISITS

Complete two additional visits with your baby’s primary care provider (PCP) when your baby is between the ages of 16-30 months.

  • Seventh and Eighth Visit Rewards: Earn a $10 gift card for each visit to buy supplies for your baby. ($20 total)

Once enrolled, please fill out this form after each well-child visit and send it to the above address.

Well-Child Visit Form

We will accept submissions for up to one year after your baby’s birth. Please allow 4-8 weeks to receive your gift card(s).

Presbyterian Health Plan, Inc. is not responsible for any lost or stolen gift cards.

CENTENNIAL REWARDS

Centennial Care will reward you for doing something good for your health. You can earn points for your good habits. If you get certain medical tests or fill certain prescriptions (drugs) that help keep you healthy you will earn reward points. Participating in our Baby Bonuses can help you earn rewards. Spend your rewards points on health, wellness and fitness items.
