To assist members, we use the most current lists of in-network providers and facilities in our online provider directories. Your health plan materials are the best source to help you know your covered services and providers.

Individual and Family Plans

Gold, Silver and Bronze plans

Network Name: Individual and Family or Group HMO/POS
for Gold 3, Silver 1, Silver 2, Silver 6 or Bronze 2 plans
Plans offered to any NM resident

Network Name: Individual Select HMO
for Gold 1, Gold 2, Gold 4, Gold+ with TytoHome, Silver 3, Silver 5 or Bronze 1 plans
Plans offered to Bernalillo, Sandoval, Valencia, Torrance, and Santa Fe County residents

Medicaid - Centennial Care

Centennial Care is a government-funded program that offers health insurance to low-income individuals, including families with children, individuals with disabilities and the elderly.

Medicare Advantage

Presbyterian Medicare Advantage plans are for individuals who are entitled to Medicare Part A, enrolled in Medicare Part B, and live in New Mexico. Some exceptions apply.

Getting Care When Outside of New Mexico

All plans cover urgent care and emergency care services wherever you are, whether you are in or outside of New Mexico.

But, your plan may or may not have coverage for non-emergency care outside of New Mexico. Please call the Presbyterian Customer Service Center using the number on the back of your ID card. You can also review your plan materials to learn more about your plan's out-of-state coverage.

Whether you have out-of-state coverage or if you are paying out-of-pocket, it's a good idea to use our national healthcare provider network. To get services outside of New Mexico, members can search for a provider through our national network of medical providers or our national network of behavioral health providers.
