Member ID Card

Download Your Member ID Card Online with myPRES

Do you need to view, replace or get a copy of your member ID card? With myPRES, you can easily access it on your phone, computer or tablet. This makes it easy for you to manage your healthcare needs anytime and from anywhere.

Important information about your member ID card

All Presbyterian Health Plan and Presbyterian Insurance Company members receive a member ID card including family members who are on their plan. The card shows that you have health insurance and it is important to have it with you when you:

  • See a provider

  • Go to the pharmacy

  • Get lab tests

  • Go the hospital

  • Are traveling, in case of emergency

Please take care of your ID card like you would a driver’s license or credit card.


Your ID Card Explained

Your ID card has information that is helpful to you and your provider.

Front

  • Member Name: Your name

  • Member ID: Your personal ID number

  • Group Number: This number is specific to your plan type or employer

  • Plan: The name of your health plan

  • Aetna logo:

    If you see this on the front or back of your card, this means you can access the Aetna provider network for care when you are outside of New Mexico. The type of care you have access to depends on your plan type.

    • Note: you are always covered for urgent and emergency care in or outside of New Mexico.

  • Phone Numbers: If you have any questions, call these numbers to reach an expert in your plan. The numbers listed will connect you to the Presbyterian Customer Service Center.

  • Submit medical claims: This information is for your provider’s office. Whenever you get care, your provider will need this information for billing.

Based on a Model of Care review, Presbyterian Dual Plus (HMO D-SNP) has been approved by the National Committee for Quality Assurance (NCQA) to operate a Special Needs Plan (SNP) through 2028.
