Do you need to replace, view, or need a copy of your member ID card? You can view, fax, or email it straight from your phone, computer, or tablet.

What you need to know about your using your member ID card

You join Presbyterian Health Plan we will send you a member ID card to you and your covered family members. Your member ID card is used a proof that you have health insurance with us. It is important to keep your member ID card with you when you:

  • Go to the doctor

  • Go the pharmacy

  • Have lab tests

  • Go the hospital

Your ID card has important information on it, you should keep it safe and treat as you would anything with your personal information on - such as your driver’s license or credit card.


Your ID Card Explained

Your ID card has information that is helpful to you and your providers.

Front

  • Member Name: Your name

  • Member Number: Your member ID number is unique to you

  • Effective Date: The date your health insurance coverage became effective.

  • Group Number: Your group number is specific to your plan type or employer.

  • Plan: The name of your health plan.

  • Aetna logo: If you see an Aetna logo on either the front or the back of your card, this means you can access the Aetna provider network if you need to get care when you are outside of New Mexico. The type of care you have access to depends on your plan type. Check your plan documents or call customer service for details.

    • Note: you are always covered for urgent or emergency care wherever you are – it doesn’t matter if you are in New Mexico or outside of New Mexico.

  • Phone Number: If you have any questions, call these numbers to reach an expert in your plan. The numbers listed will connect you to the Presbyterian Customer Service Center.

  • Submit medical claims: This information is meant for your provider. Whenever you receive care, make sure your provider has your health insurance information. This is information they need to know.

It is important to take your ID card with you when you travel. It has important information and phone numbers you will in case you have an emergency.
