MyChart – Access to Your Presbyterian Health Information

As a Presbyterian Medical Group patient, you have secure online access to your Presbyterian electronic health records through your MyChart account. You can also have access to your family's records by requesting proxy access and MyChart offers teens, between the ages of 14-17, access to directly engage with their care team. Plus, you can download the mobile app and manage your entire family's health care while on the go.

Using MyChart, you can:

  • View test results
  • Send and receive non-urgent medical messages with your care team
  • Request prescription renewals and view current medications
  • Schedule primary care appointments, request an appointment with a specialist, view past appointments, and cancel or confirm upcoming appointments
  • Use eCheck-In before your appointments to pay co-pays, sign documents, and verify and update patient information
  • View and accept newly available appointments
  • Review details of your visit or patient instructions in your After Visit Summary
  • Access your health summary including allergies, immunizations and current health issues
  • Receive notifications of appointment and health reminders

Requesting Proxy Access

With proxy access, you can view certain medical information of your children or other adults.

For Your Child's Medical Records

If you are a parent or guardian of a minor child under the age of 14 and would like to request access to their medical records, you can request proxy status.

You will be required to have your own MyChart account in order to access your child's records.

You can request access via your MyChart account. Once logged in, choose My Medical Record > Request Access to a Child. Complete the proxy access form and submit. You can also call 505-923-5590.

Below is the information you will need to have about your child and yourself in order to complete the request:

  • Full name
  • Date of birth
  • Address
  • Name of child's primary care provider
  • Last 4 digits of child's Social Security Number
  • Insurance member ID

The request will be reviewed and if determined to be valid, access will be granted and you will be notified via MyChart.

MyChart accounts for children between the ages of 14-17 can only be activated by that child. The teen can choose to grant limited view proxy access to an adult. If a parent or legal guardian already has proxy access to a child's account prior to their 14th birthday, their access will transition to a limited view proxy access on that birthday. Our policy follows New Mexico state law concerning the release of information for minors in this age range. On the child's 18th birthday, a parent or legal guardian's proxy access will be terminated. The adult patient (age 18+) can grant full view proxy access using his/her MyChart account using the Invite Someone to Have Access to Your Records link in the account.

Minors age 14-17 with Complex Medical Conditions: The parent or legal guardian will need to message or call the provider to request full view proxy access. Both the provider and the patient must agree to full view proxy access. This is to support the teen’s right to confidentiality since full view proxy access includes clinical information that may be sensitive.

For an Adult's Medical Records

If you are an adult and would like access to another adult's medical records, you can request proxy status. Adult proxies can only be requested by calling 505-923-5590 or visiting the Presbyterian Medical Group provider's office of the adult you are requesting access to.

As the requesting adult, you must have your own MyChart account in order to access another adult's records.

Information you will need to have about the adult and yourself in order to complete the request:

  • Full name
  • Date of birth
  • Address
  • Name of adult's primary care provider
  • Last 4 digits of adult's Social Security Number
  • Insurance member ID

The adult whose record is being made available will need to speak with our verification team. After that, the request will be reviewed and if determined to be valid, access will be granted and you will be notified via MyChart.

Can I use MyChart for urgent or emergency needs?

MyChart is NOT to be used for urgent needs. Please visit one of our urgent care locations or call your clinic. For medical emergencies, dial 911.

How do I access MyChart?

You will use your myPRES ID and password to access MyChart.

How do I register for MyChart?

  1. Go to myPRES Registration
  2. On the myPRES Registration page, enter your information and select Register.
  3. When the MyChart box appears, select Activate MyChart. Note: If you have not yet been seen at a Presbyterian facility but want to register for MyChart in anticipation of becoming a patient, you will need to call Customer Service at 505-923-5590 or toll free at 1-866-861-7444 to receive an activation code which you will then enter into the registration form.
  4. Read through our Terms and Conditions. If you agree to these, select Accept.
  5. You will see the home page of MyChart appear on your screen. You are now activated.

How do I get help with registration or logging in?

What do I do if I’ve forgotten my password?

If you forget your myPRES password, use the “Forgot your password” link at the Sign In page. You will be asked to answer your challenge question and then your password will be sent to your email.

What web browsers work best with MyChart?

To ensure the best experience, you will need to enable pop-ups and use a recommended web browser or mobile device:
  • Google Chrome or Mozilla Firefox web browser, if using a computer
  • MyChart app for iOS or Android
