They are required to be reviewed and acknowledged for both in-person and telehealth video appointments with your provider. If you have an upcoming telehealth video appointment and have received a text message to review these documents, please do so then reply to the text message confirming your review and consent for treatment. This is a requirement and MUST be acknowledged before your provider can treat you. If you are unable to acknowledge these documents before your appointment, your appointment may need to be rescheduled to a future date.
