Pre-Registration Forms

The documents below provide important information, guidelines and resources related to your appointment. They are required to be reviewed and acknowledged for both in-person and telehealth video appointments with your provider. Click on the links below to review these documents before your appointment so you are familiar with them.

If you have an upcoming telehealth video appointment and have received a text message to review these documents, please do so then reply to the text message confirming your review and consent for treatment. This is a requirement and MUST be acknowledged before your provider can treat you. If you are unable to acknowledge these documents before your appointment, your appointment may need to be rescheduled to a future date.

Test Your Device. If you are scheduled for a video appointment, please test your smartphone or computer before your appointment.

Your responsibility, if any, for charges related to telehealth services is determined by your insurance plan. Please check with your health insurer if you have any questions. Learn more here.
