Pediatric Chronic Care Clinic
The Pediatric Chronic Care Clinic works with children who have chronic or complex medical problems and often need to see a variety of specialists. Our clinic will communicate and coordinate care with your child’s primary care provider (PCP). When patients enter young adulthood, we will help them transition to a provider who cares for adult patients, while continuing to follow and assist in their care.

Our team can help arrange and manage visits and treatment needs to help coordinate a personalized treatment plan for your child. This team can also provide extra support for your child’s health needs, including behavioral health services.
