Pediatric Chronic Care Clinic

The Pediatric Chronic Care Clinic works with children who have chronic or complex medical problems and often need to see a variety of specialists. Our clinic will communicate and coordinate care with your child’s primary care provider (PCP). When patients enter young adulthood, we will help them transition to a provider who cares for adult patients, while continuing to follow and assist in their care.

Since coordinating office visits and treatment plans among specialists can be complicated and inconvenient, the Chronic Care Clinic can help by:

  • Providing care coordination

  • Facilitating communication and collaboration among your child's healthcare providers

  • Scheduling longer visits

  • Preparing personalized treatment plans for your child

  • Providing a physician, nurse practitioner and social worker for your child’s care
