Pediatric Chronic Care Clinic
The Pediatric Chronic Care Clinic works with children who have chronic or complex medical problems and often need to see a variety of specialists. Our clinic will communicate and coordinate care with your child’s primary care provider (PCP). When patients enter young adulthood, we will help them transition to a provider who cares for adult patients, while continuing to follow and assist in their care.

Our chronic care clinic providers include your child’s primary healthcare provider, pediatric specialists your child may see, and other trained healthcare workers such as nurses, therapists and home health aids. Our team can help arrange and manage visits and treatment needs to help coordinate a personalized treatment plan for your child. This team can also provide extra support for your child’s health needs, including behavioral health services.
