Our Pediatric Chronic Care Clinic provides special care for children with chronic medical conditions. We see children of all ages – from newborns to patients up to 21 years old.
Chronic conditions may include:

Congenital heart defects

Genetic syndromes

Cerebral palsy

Short Bowel Syndrome (SBS), also known as short gut syndrome

Premature babies with lung, eye or feeding issues
Our clinic will communicate and coordinate care with your child’s primary care provider (PCP). When patients enter young adulthood, we will help them transition to a provider who cares for adult patients, while continuing to follow and assist in their care.
